How to Submit Proof of Eligibility

 

In order to become certified with American Allied Health, one must provide documentation that he or she is eligible to sit for the online exam. Documentation should be sent after you register for the exam, but not necessarily before you take the exam: you may register and take the exam at any time, but AAH will not release hard copies of your certificate and registration card until our office has received proof of eligibility. 

The criterion for eligibility may be fulfilled in any of the following ways:

• Graduation from a vocational training program in your field of study
• One year of work experience in the field (you do not need to be currently employed)
• Military experience/training in the field
• Reciprocity from another certifying agency

The following forms of documentation are acceptable:

• Certificate of training
• Graduation diploma
• School Transcript
• Letter from employer/supervisor
• Copy of check stub showing length of employment
• Certification number and name of another certifying agency

You may submit your documentation to American Allied Health any of the following ways:

Option (a): By Postal Mail:

1. Download and print AAH's Proof of Eligibility Form.
2. Mail in the Proof of Eligibility Form along with a photocopy of your documentation to:

Option (b): By Fax:

1. Download and print AAH's Proof of Eligibility Form.
2. Fax the Proof of Eligibility Form along with a photocopy of your documentation to: (479) 553-7285.

Option (c): By Email:

1. Download AAH's Proof of Eligibility Form.
2. Email the form, along with documentation, as email attachments to: proof@americanalliedhealth.com.

Option (d): Via Smart Phone (e.g., iPhone, Android):

1. Download and print AAH's Proof of Eligibility Form.
2. Take an image capture of the completed form.
3. Take an image capture of your documentation (e.g. a picture of your certificate of training).
4. MMS or email the two images to proof@americanalliedhealth.com.

Option (e): Upload Proof Online to Your AAH Account

1. Log in to your AAH account.
2. Click on the "Edit Profile Info" tab.
3. Click on the "Edit Profile Information" link.
4. Upload PDF, .doc, or image files of documentation.
